  Physical Examination Record for Foreigners

  please i want to know exactly what you are looking for on the physical examination, i mean what test am i suppose to do, like malaria, hepatitis etc and also where can i obtain the visa application form

• the physical excamination includes many tests, but mostly are routine tests. here is a form you can refer to: http://www.chinaembassy.org.sa/eng/hzhqz/df/P020100101598125976604.pdf
  you can obtain the visa application form from the Chinese Embassy in your country or download the forms from the offcial website of the Chinese Embassy in your country.
