I am a US passport holder. How can get entry visa to China?

I was born in China, and my family moved to the US when I was very little. Then I was adopted by my parents when I was 5. Now, I would like to go back to the China but have some questions about the visa application. No one in my family knows my Chinese name as my biological parents never submitted any official documents to adoption agency. How can I fill in the other names, formal nationality and place of birth?

are you going to China for a short visit or long-tern stay?
as a US citizen now,you can apply for a tourist, study, and work visa to China.
you don't need to fill in the other names, for nationality,it should be US, place of birth, just fill in China.
