for police clearance
i work in macau before since 2005 till 2013 now i am in the philippines applaying my visa to australia and one of the requirements is to get the macau police clearance..i would like to know if where do i get the application form ?

- Susan, the application form is not necessary. You can apply via posts to Macau P.O. Box 1089.
You must submit a letter listing the following information: your full name, date and place of birth, names of your parents, nationality, marital status, purpose for the certificate request (ex: visa application at the U.S. Consulate General) andyour contact information (address and phone number).
The letter must be signed by you with the print of your right index finger visible next to the signature. (First time applicants must submit all ten fingerprints.)
You must submit documents proving your former residence in Macau (documents of identification issued in Macau or a testimonial letter from a former school or employer in Macau).
In addition, you must submit a photocopy of a relevant document showing his/her parents' names (such as a birth certificate) if the data is not already on record with the Identification Department.
You must present a photocopy of a passport issued by the country where you presently resides, or of which the applicant is a national.Answered by David (Canada) | Jul. 16, 2015 05:28
