  My passport is destroyed. Do i need go to the embassy to get a new form?

  My passport get wet, and the photo color has changed, the date i entered the u.s on i-94 is missing. do i need go to the embassy to get a new form?
  hello, i am a chinese student, i got the J-1 visa. few days ago my passport get wet, and the photo color has changed, what's worse,the date i entered the u.s on i-94 is missing. i am worry about it. so do i need go to the embassy to get a new form?
