Question about China Embassy in Canada

HK ID application forms
I want to apply for the HK ID in Toronto and although I was born in Canada, my parents have their HK permanent identity cards as they were born in HK.
I realize that I can apply through the Chinese Consulate on St. George Street, but I cannot seem to find the necessary forms (tried looking on their website).
Does anyone know where I can get the forms to start the application?
